Small hardcover with dust jacket; it's been on my shelf for quite a while. I often don't register reference books, and this one was destined to be donated to the library unregistered.

Then a couple of days ago I learned that a family friend had lost his battle with cancer. So this book is being registered and released in memory of my calabash uncle Webster: I will remember you when I next see a rose garden.
