I found this good-condition paperback at the Used Book Superstore in Burlington MA. It's a choose-your-own-path gamebook from the "Fighting Fantasy" series.
This series adds actual random-number elements to the choices; you can roll your own dice, if you have them, and if not you can open the book at random and use the dice printed at the bottom of the page. The choices range from simple "go left/right/straight" to more weighted choices, and there are encounters with foes that you must fight (or, sometimes, have the option to try other tactics on). You do have to keep track of your own stats and possessions in this game, as in a regular D&D session; this adds an element of strategy to the whole proceeding, but I admit I prefer my choose-your-own books with less bookkeeping!
Since the premise of the story is that you're undergoing a very serious and dangerous test, the various endings feature different rewards or penalties than one might expect in a typical dungeon-crawl. Lots of entertaining twists here!
