This title is misleading. It should be called something like 'How a close examination of a really outstanding cardio-thoracic surgery department demonstrates how all aspects of health care are inter-related.' Probably won't fly as a sexy title, but great book.
Author looks at how a surgery department runs, and functions. He follows surgeons, ward clerks, patients, transplant pilots everyone who will put up with him. It's a nice job of connecting how the American model of health care is fraying at the edges. No blame attached anywhere, just pressures exposed.
