2007 Random House hardcover, with covered dust jacket. This copy was a discard from Dimond Library. I found it there last week in the free books box on the porch.
Edit March 1, 2021: This was a somewhat interesting read about a man overcoming blindness. I read the first part, then skimmed to the end for time. So I think it is okay, for this specialized medical interest. I currently have this ranked at #986 on my ranked list of best-read books lately.
