It belongs to North America (even though it belongs to Denmark) because, like Russia and Turkey, it is a country that goes across two continents.
As for Scotland, it is up to you, you can include it as UK, or you can separate the British Isles into England, Ireland, Scotland & Wales...personally I'd class a Scottish author as being counted as the UK for this challenge, but its ultimately up to your own interpretation.

Europe - oodles of books, and this book really is set in 3 countries: England, Scotland AND the Netherlands - so I think I go with the AUTHOR's country :-)
A little rearranging of the 666 spreadsheet to do, but that was expected as some settings can't really be ascertained until you read the book.
